Wisdom teeth are the four third molars that sit at the very back of your mouth. They are the last teeth to develop, typically appearing between the ages of 17 and 25. In Nepali, they are commonly called akkel teeth.
The problem is simple: our jaws often do not have enough space for four extra molars. When a wisdom tooth cannot emerge properly, it becomes impacted. This can cause pain, infection, damage to neighboring teeth, and crowding.

Honest Advice First
Does Every Wisdom Tooth Need to Come Out?
The honest answer is no. Not every wisdom tooth needs to be removed.
If your wisdom tooth has erupted fully into a good position, is not causing any pain or infection, and you can clean it properly with a toothbrush, it may be fine to leave in place. Your dentist will monitor it at regular checkups.
Removal is generally recommended when:
- • The tooth is impacted or partially erupted and causing repeated infections
- • The tooth is pushing against and damaging the tooth in front of it
- • There is a cyst forming around the unerupted tooth
- • The position makes the tooth impossible to clean, leading to decay
- • Orthodontic treatment requires more space in the jaw

Types of Wisdom Tooth Impaction
An OPG (panoramic) X-ray reveals exactly how your wisdom tooth is positioned. The impaction type determines how complex the removal will be.
Soft Tissue Impaction
The tooth has erupted through the bone but the gum tissue is still partially covering it. This often causes repeated infection in the overlying gum flap (pericoronitis).
Relatively StraightforwardPartial Bony Impaction
Part of the tooth is still stuck in the jawbone. The crown has partially emerged but the tooth is angled or tilted, making a clean eruption impossible.
Moderate ComplexityFull Bony (Horizontal) Impaction
The tooth is completely buried in the jawbone and lying on its side, directly pressing against the root of the adjacent molar. This is the most complex type to remove.

Consultation & X-Ray
We take an OPG X-ray to see the exact position of your wisdom tooth and plan the right approach for your case.
Anaesthesia
Local anaesthesia is applied. You will feel pressure during the procedure but not sharp pain. The area is fully numb before we begin.
Extraction
Depending on the case, we perform a simple or surgical extraction. For impacted teeth, a small incision is made and the tooth is removed carefully in sections if needed.
Recovery & Follow-Up
You go home with aftercare instructions and any needed medications. We follow up to make sure healing is progressing well.

What to Do (and What to Avoid)
The first 24 to 48 hours are the most important for proper healing. Follow these guidelines carefully.
Do These Things
- Bite firmly on the gauze pad for 30 to 45 minutes to control bleeding
- Take prescribed pain relief as directed, even before the anaesthesia wears off
- Keep your head elevated when resting to reduce swelling
- Apply a cold pack to the outside of your cheek for the first 24 hours (20 minutes on, 20 minutes off)
- Rinse gently with warm salt water starting from 24 hours after the procedure
- Eat soft foods and keep the area as clean as possible
Avoid These Things
- Do not use a straw for at least 72 hours (suction dislodges the blood clot)
- Do not rinse forcefully or spit hard in the first 24 hours
- Do not smoke (delays healing significantly and increases dry socket risk)
- Do not eat hard, crunchy, or chewy foods until fully healed
- Do not poke the socket with your tongue or finger
- Do not take aspirin unless prescribed (it thins the blood and can increase bleeding)
Diet After Extraction
What Can I Eat After Wisdom Tooth Removal?
Good nutrition supports faster healing. Here is what to eat at each stage of recovery.
Very Soft Only
- Plain dahi (yogurt)
- Soft khichdi
- Smooth soup or dal (cooled)
- Mashed banana
- Ice cream (no cone)
Avoid anything hot as it can increase bleeding.
Soft but More Variety
- Bhat (soft cooked rice)
- Boiled vegetables, well-mashed
- Dal with soft roti
- Scrambled eggs
- Smooth porridge (dhido)
Chew on the opposite side of the extraction.
Returning to Normal
- Regular bhat and tarkari
- Soft chicken or fish
- Cooked lentils
- Gradually reintroduce firmer foods
Avoid hard biscuits, nuts, or popcorn until fully healed.
Know the Signs
Possible Complications to Watch For
Complications after wisdom tooth removal are uncommon, but knowing what to watch for helps you act quickly if something is not right.
🕳️ Dry Socket (Alveolar Osteitis)
Dry socket is the most common complication. It occurs when the blood clot in the extraction socket is dislodged or dissolves too early, exposing the bone underneath. It causes a sharp, aching pain that starts 3 to 5 days after extraction. The good news: it is very treatable. Contact us and we will dress the socket to relieve pain quickly.
🦠 Infection
Signs of infection include worsening pain after day 3 (rather than improving), increasing swelling, fever, or a bad taste and discharge from the socket. If you notice any of these, contact us promptly. Infections are treated with antibiotics and sometimes a return visit to clean the site.
⚡ Temporary Numbness
Lower wisdom teeth sit close to the inferior alveolar nerve. In some cases, temporary numbness or tingling in the lip, chin, or tongue can occur after a lower extraction. This is usually temporary and resolves on its own over weeks to months. Permanent nerve damage is rare but is something your dentist will discuss with you before a complex lower extraction.
😬 Trismus (Difficulty Opening Mouth)
Some swelling and stiffness in the jaw muscles is normal after a surgical extraction. This usually improves within a week. Gentle jaw exercises and warm salt water rinses can help. Persistent trismus beyond 2 weeks should be reported to your dentist.

- Fever above 38°C (101°F) in the days following extraction
- Swelling that is spreading toward your throat or neck
- Severe pain that is getting worse rather than better after day 3
